Abstract
We sought to determine the point-of-care information PACT members need to successfully coordinate care, and test the effectiveness of a of a team-based audit-and-feedback intervention at improving performance on coordination indicators. Seven indicators of coordination were identified. Point-of-care information needs centered on two main areas: Patient information, and common understanding within the team and between teams and patients. The feedback intervention significantly changed performance for intervention teams in 2 of seven indicators; this effect was moderated by strength of dose and multiple team membership.
